As the practice Care Co-ordinator, I provide assistance to any patients who are in need of extra support, to help them to remain living at home as independently and happily as possible, for as long as possible. A large part of my role is as a liason between patients and other staff at the surgery if needed, and also with any community organisations as appropriate I also support patients who are carers for family members, friends etc, and who may be experiencing difficulties in their role I work every day except Mondays, so if you feel I may be able to help you, please do contact me at the surgery, and I will do my very best for you.
