Our care co-ordinator is Helen. The aim of care-co-ordinator is to signpost and empower patients to engage in community services. Helen can liaise with outside agencies/charities/services on their behalf. Helen can provide information and advice on all aspects of patients social needs. Helen is available from Wednesday - Friday 2pm - 18.30pm.
